Doctors and hospitals you already use

Write down the doctor, the office, the medical group, and the hospital you would use. Neighbors often name Huntington in Pasadena, USC Arcadia Hospital, City of Hope in Duarte, Cedars-Sinai, and UCLA Health. A name here is not a network list.
