Medicare Part D 2027

Some 2027 Part D numbers are already published. The yearly cap, the standard deductible, and the base premium are not the premium you will pay. This page does not pick a drug plan.

The numbers CMS has published

In 2027 the annual Part D out-of-pocket threshold is $2,400, up from $2,100 in 2026. After you reach that amount for covered Part D drugs, you generally pay $0 for those drugs for the rest of the year. The standard maximum deductible rises from $615 to $700. A plan can use a lower deductible. The 2027 base beneficiary premium is $41.33. That is a formula number, not your bill.

What is not your premium

The premium you pay depends on the plan, the pharmacy, and whether Extra Help or IRMAA applies. The temporary premium stabilization demonstration ends after 2026, so a premium can change even when the cap goes up. Read the Annual Notice of Change. Compare the drugs, not only the premium, on the Part D plan list.

Extra Help still changes the math

Extra Help, the Part D Low-Income Subsidy, can lower a premium, the deductible, and copays for people who qualify. Social Security decides that. This site cannot. Use the Extra Help screener as a sketch, then apply at Social Security. A late Part D penalty can also be waived for someone with Extra Help.

The rest of the 2027 changes

Negotiated prices for another 15 Part D drugs take effect in 2027. The Medicare Prescription Payment Plan continues. It spreads what you owe. It does not lower it. Part B’s 2027 premium is not final. The wider list is in Medicare changes for 2027.
