Inside the United States
Original Medicare pays its share for a provider anywhere in the country who accepts Medicare and is taking patients. A Medigap policy then helps with some of the costs Medicare does not pay. It does not create a network. A Medicare Advantage plan has a service area. Emergency and urgently needed care away from home are not the same as a routine visit. A PPO may pay some out-of-network care and still leave a higher bill. A PPO is not Original Medicare.
Outside the United States
Original Medicare usually does not pay for care outside the United States. Some standardized Medigap letters include a foreign-travel emergency benefit: a deductible, then a share of the emergency bill, only in the first part of a trip, and only up to a lifetime dollar cap. The outline of coverage states the amounts. It is not travel insurance, and it is not a reason by itself to choose a letter. Some Medicare Advantage plans offer a travel extra. That extra is not standardized. Read this year’s Evidence of Coverage. More detail is in Medicare outside the United States.
Snowbirds and months away
If you live part of the year outside a plan’s service area, routine care can be the problem, not the emergency. Ask whether a checkup, a specialist, or a refill is covered where you will actually be. People who want doctors in two states often start with Original Medicare and then ask what a Supplement would pay. People who stay inside one county and rarely travel often start with the local network. Original Medicare or Medicare Advantage? is the structure comparison.
What to write down before you compare
List the months you are away, the states or countries, and whether the care would be routine or an emergency. Then list the doctors you still want at home. A travel benefit does not keep a local doctor. Keeping your doctors is that check. Talk with Alan if you want the travel months and the doctor list looked at together.
Common questions
- Which is better for frequent travelers, Medicare Advantage or Medigap?
- Neither is automatically better. Original Medicare can be used with providers who accept Medicare anywhere in the country. A Medicare Advantage plan usually limits routine care to a service area. Emergency rules are different from routine care. Your months away and your doctors decide the fit.
- Does Medigap cover a trip outside the United States?
- Some letters include a foreign-travel emergency benefit with limits. It is not full travel coverage, and Original Medicare itself usually pays nothing abroad. Read the outline of coverage.
- Does a PPO work like Original Medicare while I travel?
- No. A PPO may pay some out-of-network care and can still cost more. It is a Medicare Advantage plan, not Original Medicare.
