My Medicare Checkup, scheduling, contact, and privacy-request forms collect: name, ZIP code, phone, email, checkup answers (coverage situation only), appointment preferences, and your TCPA/SMS consent choices. We do not collect Social Security numbers, Medicare IDs, diagnoses, or medication names on this website.

When you submit a form, we create or update a contact record in HubSpot with your name, email, phone, ZIP (when provided), the form you used, checkup answers (coverage situation only), appointment preferences, and your TCPA/SMS consent choices. We use HubSpot so a licensed Medisure Agency advisor can follow up. We do not email form contents to a public inbox. We do not sell or share this information with other agencies as a lead.

We use that information to respond to your request, schedule a review, and — if you consented — to contact you by phone, email, or SMS. We keep a consent receipt in your browser as a backup. Advisors should retain TCPA consent records in HubSpot for at least five years.
