Hoag medical groups are leaving SCAN’s network

SCAN told brokers that Hoag Medical Group and Hoag Physician Partners will no longer be in SCAN’s provider network effective December 31, 2026. This page is the public version of that notice. It is not a reason to leave or stay, and it is not an enrollment.

Which groups the notice names

The notice names two groups, not every doctor who practices at a Hoag hospital. Hoag Medical Group is IPA ID 1105. Hoag Physician Partners is IPA ID 1099. SCAN said it could not reach an agreement with those groups. A hospital name on a building is not the same as one of these medical groups. Ask the office which group bills your visits.

The dates in the notice

Use the dates SCAN gave brokers. Confirm them with SCAN or the office if your letter says something different.

  • October 1, 2026: SCAN said it will close the panels, so new enrollments should not be added to these two groups.
  • Week of October 5, 2026: SCAN said impacted members start receiving an advance letter about the network end.
  • About 45 days before the end date: SCAN said a reminder letter will be mailed.
  • December 31, 2026: SCAN said these groups will no longer be in its provider network as of this date.

What SCAN said it will do for current members

SCAN’s notice says affected members will be reassigned to a new network and will keep the current primary doctor when that doctor is available. If that doctor is not available, SCAN said the member will be assigned a different primary doctor. The notice also says a member may change the medical group or primary doctor by calling SCAN Member Services at (800) 559-3500 (TTY 711). A reassignment on paper is not proof the new doctor is the one you want. Call the new office before you rely on it.

If you want to keep the Hoag doctor

SCAN’s notice says people who want to keep seeing a Hoag primary doctor should work with Hoag to understand their options. Ask that office which 2027 plans, if any, will still include that doctor and that medical group. Then check the same doctor, the same office address, and the same plan names on Medicare.gov. “We take Medicare” does not mean a SCAN plan will still pay that office after the end date. This lands during Annual Enrollment

Annual Enrollment for 2027 coverage is October 15 through December 7, 2026. A change made in that window generally starts January 1, 2027, which is the day after the date in SCAN’s notice. Doing nothing does not freeze 2026. SCAN described a reassignment inside its own network. Switching to another company, or back to Original Medicare, is a separate decision with its own drug and Supplement rules. The dates are on the 2027 Annual Enrollment page. Reviewing your alternatives

If this notice names your medical group, Talk with Alan to review your alternatives. Have your SCAN letter, your doctor’s name, and the medical group with you. Alan will go through the reassignment SCAN described, whether that Hoag doctor will still be available on another plan, and what you would need to decide before January 1. Talking with Alan does not enroll you, and you do not have to make a change.

What this page will not do

There is no member list here. SCAN told brokers a list would be placed on a producer portal. That list is not a public directory, and this site will not publish it. This page also does not say that another company will include Hoag, and it does not say you should leave SCAN. The check is the named doctor, the named group, and the plan that will pay them in 2027.

When do these Hoag groups leave SCAN?

Will SCAN assign me a new doctor?
